	WGTE Public Media - Release Form I hereby give my absolute and irrevocable permission to The Public Broadcasting Foundation of Northwest Ohio (WGTE Public Media, "WGTE") to use my materials (the "Materials") whether videotape, photographs, audio recordings, and/or otherwise record and disseminate in whole or in part my likeness and participation ("Appearance") in this submitted production to WGTE, given on the ____day of_______________________, 20___ . I hereby grant to WGTE and its authorized agents, vendors, employees, licensees, successors, affiliates and assigns (the "WGTE Parties") the non-exclusive, perpetual right and license (including the right to sublicense) to use, reproduce, display, perform, create derivative works of and distribute my Appearance and any graphics, photos, text, music, video or any other materials provided in connection with my presentation (the "Materials"), in any manner or media, in whole or in part, including without limitation for broadcast, cablecast, streaming video via Internet, or any other distribution methods available now or in the future. Except with respect to the license granted above, I retain all rights, title and interest in my Appearance and the Materials, except for the recording of my Appearance and materials submitted to WGTE, which shall be exclusively owned by WGTE. I understand and agree that I will not have any right of approval or receive any monetary compensation from WGTE or any WGTE Party as a result of any use of my Appearance or the Materials. I also consent to the publication by WGTE and the WGTE Parties of a summary or transcript of my appearance and/or comments and to the use of my Appearance, voice and biographical information in any manner, in whole or in part, in any medium and for any purpose whatsoever in perpetuity, including without limitation in the promotion of WGTE, its programs and services, without any further obligation or compensation to me. I represent and warrant that I have permission to use any Materials incorporated into or used in connection with my appearance. I also represent and warrant that use of my Appearance or the Materials under this release shall not infringe upon or violate the rights of any third party. Should use of my Appearance or the Materials by WGTE or any WGTE Party require written consent from a third party, I agree to obtain those written consents and to provide WGTE with copies of such consents in a timely manner. I further expressly release, waive, discharge and agree to indemnify and hold harmless WGTE, the WGTE Parties, and each of their agents and successors, affiliates and assigns from any and all causes of action, which may arise out of or relating to the use of my appearance or the Materials, including but not limited to claims for or relating to defamation, violation of privacy or publicity rights, confidentiality, copyright or otherwise.
